About Your Role
Children and young people in out of home care (OOHC) often have high and unmet health needs and are more disadvantaged and vulnerable than other children.
NSW Health provides coordinated health assessments and intervention for children and young people entering statutory OOHC who are expected to remain in care for longer than 90 days. The program also focuses on the needs of young people expected to leave care within the next year.
The program which coordinates this process is called The OOHC Health Pathway program, and is an outcome of the Justice Wood, Special Commission of Inquiry into Child Protection in NSW. The OOHC Health Pathway ensures children entering out of home care are provided with a comprehensive health and developmental assessment and health management plan. The program aims to support earlier identification of health and wellbeing issues for children and young people in OOHC, providing timely intervention when required. The overall aim is to improve the short and long-term health, development and wellbeing outcomes for children and young people in OOHC, as well as support carers and placement stability.
The position will work closely with MLHD OOHC Coordinators and coordinate referrals and assessment outcome reports for children and young people referred to the OOHC Health Assessments model pathway. The MLHD OOHC Health Pathways Clinician will also focus on implementing and finalizing leaving care plans with a focus on health needs for young people who are anticipated to leave care within the next year.
This position has responsibility to provide comprehensive psychosocial assessments and reports with referral recommendations and plans for children and young people in OOHC. The position will provide advocacy for these young people as well as for their carers and/or caseworker.
The position will act as a liaison and resource person to support multidisciplinary clinicians, Service Managers and other key stakeholders with the implementation of the OOHC Health Assessments pathway. The position holder will be required to work closely with child protection and sexual assault services in the MLHD, as well as working in partnership with other services within MLHD.
The position will prioritise key partnerships with other services based on the needs and priorities within the scope of the position.
The OOHC clinician will also provide consultation, support and education and training for health workers and other key stakeholders on the Health Management Pathway and processes.
